In-Home Health Assessments for Medicare Members
A Nurse Practitioner from our partner, Matrix Medical Network, can spend time with you to answer any and all questions about your health, medications or chronic conditions. Schedule your home visit, provided at no cost to you, by calling 1.888.568.1750. In addition, the Nurse Practitioner will perform simple exams like checking your blood pressure and heart rate and the information you share will be given to your doctor to help better manage your health.
